Experts say zero never events are possible

Some experts say never events such as wrong-site operations could be a thing of the past, but proper preparation and communication is crucial to eliminating such mistakes.
 
According to an opinion piece in Outpatient Surgery, the process of eliminating so-called “never events” should start before the procedure does, beginning with proper communication during patient handoffs and marking surgery sites before beginning an operation.
 
Another crucial factor, the story says, is making sure the first patient of the day gets in and treated on time. As the patient load increases during the day and gets backed up, stress builds up that can lead to mistakes along the chain of staff that deal with patient safety.
