Drug dosage errors as a result of confusion when healthcare providers incorrectly convert patient weights from pounds to kilograms are common occurrences according to a recent report in Modern Healthcare.

As a result, the ECRI Institute recommends that healthcare providers switch to the metric system to prevent further medication errors, according to the report.

The errors can result from simple human error, inconsistent use of one measurement system, and electronic health records (EHR) systems that can incorrectly convert a metric number into pounds.
