Oregon finds pharmacists reluctant to use reporting system

In2008, the Oregon Patient Safety Commission implemented an adverse event reporting system that provided a confidential and voluntary. However, The Oregonian reports that while nursing homes and hospitals use the system, pharmacists do not.

 No other states have pharmacy event reporting systems, though others do have state pharmacy boards. Pharmacies say that safe protocol is already in place, and they are already reporting to the state
