A column in the Centre Daily Times explains the importance of medication reconciliation to patients. The Daily Times is the Pennsylvania State University newspaper.

The column, written by Mount Nittany Medical Center's patient safety officer, asks patients do their best to maintain an up-to-date list of medication they are taking. The hospital is doing its part by providing wallet-sized cards that patients can use to record their medications and keep with them at all times.

Other information, like allergies, should also be recorded. The hospital plans to distribute the cards to all its patients and to local physician offices, according to the column.
