    The success of a cause analysis program (CAP) requires careful management of cause analysis quality. The responsibility for quality must start with the team and the team lead; they need to perform a high-quality analysis and recommend meaningful corrective actions. The process owner is responsible for ensuring the quality of the analysis before it is passed on to the oversight committee, which is ultimately responsible for the quality of the program that generates the results and corrective actions. The responsibility for the quality and meaningfulness of the corrective actions portfolio should rest with the oversight committee.

    We’ve provided a quick checklist from Ken Rohde’s book Beyond Root Cause Analysis: Building an Effect Program, that can be used by all levels of your team to evaluate the quality of root cause analysis (RCA) and action plans.

    The A3 report is a common tool used in Lean methodology to make processes more efficient, effective, and safe.
